Overview: The primary role of our behavioral health case manager is to identify the care needs, develop plans of care and coordinate the continuity of care. Our challenges are:

  • Assure that health and safety will be met.
  • Identify individuals at highest risk as early as possible, and provide intervention services and linkages in an appropriate environment
  • Assure that the individual gains access to needed medical, social, education, transportation, housing, nutritional, and other appropriate services
  • Identifies the need for, type of, level and intensity of services required
  • Review the delivery of appropriate services
  • Encourage the use of various community resources through referral to appropriate traditional and non-traditional providers
  • Coordinate all medical Medicaid services available

How do mental health issues affect the body and brain?

These disorders can profoundly disrupt a person's thinking, feeling, moods, ability to relate to others and capacity for coping with the demands of life. Problems with mental health can affect persons of any age, race, religion, or income. Mental health-related illnesses are not the result of personal weakness, lack of character, or poor upbringing.

Mental health-related illnesses are treatable. Most people with serious illness need medication to help control symptoms, but also rely on supportive counseling, self-help groups, assistance with housing, vocational rehabilitation, income assistance and other community services in order to achieve their highest level of recovery.
